Nursing Services

Paonia Care and Rehabilitation Center provides 24 hour skilled nursing care to meet the individual needs of each resident. We provide nursing coverage with Registered Nurses (RN's) and Licensed Practical Nurses (LPN's) as well as Certified Nursing Assistants (CNA's). We have access to four physicians who reside in the community and are on call 24 hours per day. Also available is the North Fork Ambulance Association with two ambulances less than one mile away.

Our facility works in cooperation with hospitals in Delta, Montrose, and Grand Junction to ensure that all medical and nursing needs are met. We have established relationships with specialists in surrounding communities who provide all necessary medical needs. Transportation to and from appointments both in Paonia and other communities is available with the assistance of our EMT/CNA, trained transport aid, and the use of our wheelchair accessible van.

Our Nursing team also includes an advanced Restorative Program. The goal of the Restorative Program is to ensure that all residents will achieve and maintain their maximum ability. Restorative Therapy programs include range of motion, assistance with activities of daily living, an ambulation program, and a dining program. All of these are offered are on a group and/or an individual basis, depending on the needs of each resident. All Restorative Therapy is provided by specially trained Restorative Nursing Assistants. Restorative Therapy works very closely with the Physical Therapists, Occupational Therapists, and Speech Therapists to ensure the highest quality of care is provided on a consistent basis.
